Objectives

The session will explore how the NHS has evolved over time, how national and local organisations interact, and how financial decisions impact clinical services. Participants will gain insight into the financial pressures facing the NHS and how different payment systems, contracts, and costing approaches influence service delivery and decision-making.

Learning outcomes

On completion of the session, resident doctors will be able to:

  • Describe the current NHS structure and the role of key organisations relevant to clinical practice
  • Explain, at a high level, how NHS funding is allocated and flows through commissioning arrangements
  • Identify key financial challenges facing the NHS and relate these to pressures in clinical settings
  • Outline the main NHS payment systems and contractual models
  • Demonstrate a basic understanding of NHS costing principles, including an overview of Patient Level Information and Costing Systems (PLICS)
  • Recognise when a business case is required and understand the key elements involved in developing a simple NHS business case
  • Apply this knowledge to engage more effectively in service development, quality improvement, and resource-related discussions
