Aims

To finish the day able to deliver a gold standard primary survey, identify common injuries in major trauma, initiate emergency interventions and communicate effectively in a major trauma team.

Objectives

Manage complex communication in time limited environments.

Correctly identify clinical emergencies, initiate management, request senior support

Recognise & initiate management of the critically ill patient; patients in peri-arrest situations

Deliver immediate therapy to an acutely ill patient.

Record & act on changes in physiological status, anticipate & plan appropriate action to prevent deterioration. Communicate with the patient, relatives & carers appropriately

Reassess acutely ill patients to monitor efficacy of interventions, including those aimed at managing acute mental illness Maintain patient safety & safety of others

Obtain relevant history, including mental health, in time limited environments:

outpatients/GP clinics/ED

Perform primary review of new referrals within the hospital or outpatient clinic; review initial diagnosis; plan appropriate strategies for further investigation

Refine problem lists; plan appropriate strategies for further investigation & management

Communicate management plan & request/instruct other healthcare professionals

Prescribe medicines, blood products & fluids correctly, accurately & unambiguously in accordance with GMC & other guidance using correct documentation to ensure that patients receive the correct drug via the correct route at the correct frequency and at the correct time

Administer blood products safely

Follow steps to minimise transfusion reactions Demonstrate awareness of/follow guidelines/protocols on safe cross matching & the use of blood/blood products

Prescribe blood/blood products appropriately, with support from haematology specialists

Recognise & manage transfusion reactions, report according to local/national guidelines

Recognise the importance of religious/cultural beliefs relating to blood products

Assist other colleagues with more complex procedures, eg:

Primary Survey (trauma A-E assessment), airway management, chest injury management, blood transfusion in trauma, using a pelvic binder, kendrick splints, trauma team communication, an AMPLE history.
