Introduction into laparoscopic right hemicolectomy and a sigmoid colectomy

Objectives

Objectives

You will learn the steps and risks of a right hemicolectomy and a sigmoid colectomy, along with splenic mobilisation.

We will cover three approaches to the right colon and also three approaches to splenic flexure mobilisation.

Familiarisation of the steps of these two procedures and laparoscopic skills and techniques required to be able to start performing parts of this operation in your institution.

A. Laparoscopic right hemicolectomy

Expected learning outcomes

- Learn the basic concepts and methodology of Hierarchical task analysis

- Understand the surgical anatomy - blood supply, dissections planes, neighbouring structures and variations

- Procedural steps

o Exposure – theatre set up, patient positioning, access/port placement, operating field

o Vascular – dissection of ileocolic pedicle and right/middle colic vessels, mesenteric dissection

o Mobilisation of the right colon segments (medial/lateral/inferomedial), surrounding structures (duodenum/ureter)

o Specimen extraction and anastomosis – extracorporeal/intracorporeal

o Safe closure of the ports and access incisions

- Adaptation to variation and unexpected events

B. Laparoscopic anterior resection

Expected learning outcomes

- Learn the basic concepts and methodology of Hierarchical task analysis

- Understand the surgical anatomy - blood supply, dissections planes, neighbouring structures and variations

- Procedural steps

o Exposure – theatre set up, patient positioning, access/port placement, operating field

o Vascular – dissection of inferior mesenteric vessels, mesenteric dissection

o Mobilisation of the sigmoid, DC, splenic flexure and rectum (medial/lateral/inferomedial), surrounding structures (gonadal vessels/ureter/pancreas/autonomic nerves/spleen)

o Specimen extraction and anastomosis – stapled and hand sewn

o Safe closure of the ports and access incisions

- Adaptation to variation and unexpected events

- TME is not covered in this course
