DescriptionThis half-day interactive session is designed for resident doctors to provide a practical introduction to how the NHS is structured, funded, and commissioned. The session will explore how the NHS has evolved over time, how national and local organisations interact, and how financial decisions impact clinical services. Participants will gain insight into the financial pressures facing the NHS and how different payment systems, contracts, and costing approaches influence service delivery and decision-making.

Objectives

By the end of the session, you will:

  • Understand how funding flows through the NHS at national, regional, and local levels
  • Recognise the role of commissioning and finance in shaping clinical services and patient care
  • Develop awareness of the financial pressures facing the NHS and the impact on clinical decision-making
  • Be introduced to key NHS payment systems, contracts, and costing approaches relevant to service delivery
  • Gain an introduction to the purpose, structure, and key components of NHS business cases
Learning outcomes

On completion of the session, you will be able to:

  • Describe the current NHS structure and the role of key organisations relevant to clinical practice
  • Explain, at a high level, how NHS funding is allocated and flows through commissioning arrangements
  • Identify key financial challenges facing the NHS and relate these to pressures in clinical settings
  • Outline the main NHS payment systems and contractual models
  • Demonstrate a basic understanding of NHS costing principles, including an overview of Patient Level Information and Costing Systems (PLICS)
  • Recognise when a business case is required and understand the key elements involved in developing a simple NHS business case
  • Apply this knowledge to engage more effectively in service development, quality improvement, and resource-related discussions
