Provide insight into the background of regional projects focused on initiating steps to move away from the current blame culture to a fair, learning and just culture.

Speaker Biography 

Jyoti Sumel

Dental Therapy Foundation Training lead EoE Dental Therapist CDS-CIC

I am a Dental Therapist working clinically across three sites for Community Dental Services CIC Leicestershire, treating paediatric, special care, and anxious patients. I also serve as the East of England Dental Therapist Foundation Training Programme Manager. Since graduating in 2016 and completing Vocational Training in 2017, I’ve expanded my scope through inhalation sedation training and leadership roles.

From 2021–2023, I was an NHSE Midlands Clinical Fellow, leading ‘Project Sphere – Patient Safety’ with Jason Wong MBE, Chief Dental Officer for England. This initiative focused on embedding a positive patient safety culture through stakeholder engagement and practical tools like huddle sheets. I am passionate about integrating human factors into dentistry and currently serve as the Human Factors Patient Safety Officer Council Member for BADT and involved with Human Factors in Dentistry: Patient Safety Sciences.

Aims

To enhance understanding of the human factors contributing to patient safety events in dentistry, promote effective team communication and reflective learning through accessible reporting mechanisms and huddle sheets, and support the transition towards a fair, learning, and just culture.

Objectives

Identify key human factors that contribute to patient safety events in dental practice and understand their impact on clinical outcomes.

Provide insight into current reporting mechanisms and regional safety initiatives, and encourage use of these available resources.

To provide delegates with supportive tools and reflective learning approaches that enhance their ability to respond to and learn from patient safety events in dental practice.
